Thank you for your interest in my practice. At this time I am no longer taking new patients. If you are looking for a psychologist to assist you options include contacting the local psychological association for names of possible psychologists, asking your primary care provider or other provider for referrals, or checking with your insurance company for providers who are in network for your plan. Thank you.
Fees per session are directly related to your insurance policy copayment and deductible. This information is often found on the card issued by your insurance company.
Payment methods accepted include cash, checks, debit cards, and all major credit cards (Visa, Mastercard, American Express, Discover, JCB, and Union Pay.)
Health Savings Account (HSA) and Flexible Spending Account (FSA) cards are also accepted.

Please bring your insurance card to the first appointment. Your co-payment, deductible, and the number of sessions allowed per year depend on your particular plan.
If you are not on these plans, or if you elect to pay out-of-pocket, you will need to pay at time of service. I provide a receipt after each session that you can submit for out-of-network reimbursement through your insurance company. (I would suggest that you verify the out-of-network reimbursement rates with your insurance company first.)
I am a Medicare Part-B provider (Novitas). I can bill your supplemental insurance provider, even if I am not in the network of your secondary insurance.
I am NOT a provider for Medicaid. If your secondary payer is Medicaid (and not a supplemental insurance policy) I will need to refer you to another provider.
Please bring your Medicare card with you to the first session.
EMPLOYEE ASSISTANCE PLANS (EAP)
I am an Employee Assistance Plan (EAP) provider with:
If your benefits include EAP sessions, these visits are no cost to you. The number of visits depends on your employer’s plan (usually EAP benefits range from 1 to 8 sessions). EAP visits may be used for assessment, brief treatment, and transition to ongoing therapy under insurance as needed.
If your EAP source provides an authorization and referral please bring a copy of this with you to our first session.
Please provide at least 24 hours advanced notice for any cancellation. No-shows without 24 hour notice for missed appointments will be charged a fee. No-show fees are generally not covered by insurance policies.
